Unconscious bias - Attitudes and beliefs that occur outside of our conscious awareness and control. Unconscious biases are learned stereotypes that are automatic, unintentional, and deeply ingrained, and able to inadvertently influence decisions and behavior.

Explicit bias - Attitudes and beliefs we are consciously aware we have about a person or group and behaviors are conducted with intent.

Discrimination - The unjust, unequal, or prejudicial treatment toward an individual or group.

Harassment - Unwelcome and sometimes unlawful conduct that demeans, insults, and offends an employee.

Categories: age/generation, race/ethnicity, color, gender identity or expression, sexual orientation, personality, maternity, parental status, religion, ancestry, marital status, arrest and court records, horn effect, halo effect, affinity, attractiveness, weight, disability, etc.


One person from your group will be filling out this online form due to your group discussion. All groups maintain their anonymity as there are no names or email addresses connected with submitted forms. Your honest anonymous answers will be given to the management team allowing them to be aware of areas of concern if there are any. Your honesty allows them to make decisions that create a safe, respectful, inclusive, fair, and productive work environment.

Please do not include names of individuals involved whether it’s management or a colleague as you discuss workplace biases you have witnessed, experienced, or perpetrated. Only share experiences.
